The City of Citrus Heights is dedicated to compassionately supporting unsheltered individuals and families while upholding our steadfast commitment to the health and safety of the entire community. Beginning in October 2023, the city partnered with the Sacramento County Department of Homeless Services and Housing (DHSH) to deliver a Street Outreach Program for unhoused individuals in Citrus Heights. The program is operated by Community HealthWorks, a nonprofit that addresses some of the most complicated issues and removes barriers so that all people have an opportunity to be healthy and lead a life of dignity and humanity.
Community HealthWorks provides the City with a full-time Community Health Worker (CHW). The CHW's role is to connect unhoused individuals with a range of supportive services, including emergency shelter, interim and permanent housing, supplies, and other resources that can help them take the next steps towards becoming housed.
Connecting Individuals to Services
A Community Health Worker (CHW) can connect individuals to services and resources. To begin the intake process, meet a CHW at one of these locations:
- Monday onsite at Living Water Church (7605 Lauppe Ln) from 1 PM to 3 PM. This location is for client intake, assessment, and services.
- Wednesday, Thursday, and Friday a CHW is available as needed. An individual may work directly with a CHW. Call 2-1-1 or press 8 for homeless services.
